Women's Moral Agency in Perinatal Care
Inequality Arising from the Neglect of Maternal Well-Being in the Spanish Health are System

This article examines how the increasing medicalization of reproductive processes, combined with the neglect of women’s emotional and relational well-being, generates structural forms of reproductive injustice within the Spanish healthcare system. It explores how the biomedical reduction of pregnancy, childbirth, and the postpartum period constrains women’s moral agency, pathologizes distress, and reinforces gender inequalities. Drawing on frameworks of reproductive justice, relational justice, and care ethics, the article proposes an ethics of accompaniment that integrates narrative capacity, relational autonomy, and interdisciplinary care as key elements for developing a more just, human, and equitable clinical practice in perinatal health.
